CACI conditions
Conditions AMEs Can Issue: when all CACI criteria are met and you are otherwise qualified, your AME may issue a regular medical certificate without a Special Issuance. Review the FAA guidance and bring the records specified by the worksheet. If criteria are not met, additional FAA review may be needed.

02 · SI / AASI
Special Issuance (SI)
FAA authorization comes first. Special Issuance allows the FAA to consider certification for certain medical conditions that do not meet the usual standards. AASI (AME Assisted Special Issuance) lets an AME reissue a certificate when the FAA’s authorization and condition-specific requirements allow it.
This directory lists the FAA’s 37 selected AASI conditions, not every diagnosis that may require SI. Follow the linked guidance for class restrictions, initial certification, required reports and renewal criteria. A guidance document is not necessarily a fill-in worksheet.

Some pathways need an additional specialist.
An AME performs the FAA medical exam; a Senior AME is required for First Class. A HIMS AME has additional FAA training for specific certification pathways. A HIMS-trained neuropsychologist performs specialized evaluation and testing. These roles are not interchangeable.
- ADHD
- Standard Track requires an FAA HIMS-trained neuropsychologist, not automatically a HIMS AME. See the ADHD guide →
- Antidepressant treatment
- The FAA Antidepressant Protocol’s initial-certification pathway for continued qualifying medication use requires a HIMS AME. Not every mental-health history uses this pathway. FAA protocol ↗
- Alcohol / substance-use pathways
- FAA-directed HIMS evaluation or monitoring may require a HIMS AME. Requirements depend on the history and FAA instructions; not every reported incident has the same pathway. FAA drug and alcohol guidance ↗
- Other conditions / Special Issuance
- Special Issuance does not automatically require a HIMS AME. The condition-specific FAA protocol or authorization determines the required specialist and follow-up.
Confirm the required designation before arranging testing. Our doctors are described here as Senior AMEs, not as HIMS AMEs. Ask the office which services and outside evaluations your case may need.
